The key to good job placements and the best pay is linked directly to how well you provide all the required employment documentation (that most facilities will require) and keeping them current. These requirements are needed for your health and the health of others. As cumbersome as it is, we try to make it as convenient and as easy as possible for you by using state of the art online software for you to utilize. This software will allow you to search pending jobs, manage your schedule, take your annual tests, remind you of things that need to be renewed and when you are out of compliance. It then allows us to verify and oversee that you have everything you need so we can submit you to facilities that need your skills. We also will be able to send those documents to anyone for you once you are in our system. Step2: We will review your application set up your profile and send you our welcome email that will include links to log-on to retrieve and sign our documentation and do any necessary testing to bring you into compliance if needed: (All this can be done automatically on-line. We then submit your profile to places that you indicate that you are interested in working. You will then usually be asked to attend an orientation to work get signed off and start working.

For you review here is the list of things that we need to have, how we need them and what needs to be kept current in our files so that we may be able to place you right away:
At least 1 year minimum experience in any field you wish to be placed within the last three years.
Application - A completed and signed APN application.
W-4 - A completed and signed W-4 for the current year.
I-9 - A completed and signed current I-9.
SSN Card - a copy front and back of your social security card that is verified.
Drivers License or State ID - Copy of front and back.
Resume/Profile - Current resume or history profile.
Two References - At least two Verifiable references.
License / Certifications - Copy of front and back (must be issued by the State that you are working in).
BLS - (Basic Life Support) Copy of front and back with signature. For first assignment, if certification expires before end date, traveler must schedule a renewal course and provide a receipt for the class. Accepted cards are Healthcare Provider-American Heart Association, Professional Rescuer-American Red Cross, Level C-Canada, or AHSI-American Heart and Safety Institute. Online certifications and renewals are not accepted.
ACLS - (Advanced Cardiac Life Support) Copy of front and back with signature. For first assignment, if certification expires before end date, traveler must schedule a renewal course and provide a receipt for the class. Accepted cards are Healthcare Provider-American Heart Association, Professional Rescuer-American Red Cross, Level C-Canada, or AHSI-American Heart and Safety Institute. Online certifications and renewals are not accepted.
NRP - (Neonatal Resuscitation Program) Copy of front and back with signature. For first assignment, if certification expires before end date, traveler must schedule a renewal course and provide a receipt for the class. Online certifications and renewals are not accepted.
PALS - (Pediatric Life Support) Copy of front and back with signature. For first assignment, if certification expires before end date, traveler must schedule a renewal course and provide a receipt for the class. Accepted cards are Healthcare Provider-American Heart Association, Professional Rescuer-American Red Cross, Level C-Canada, or AHSI-American Heart and Safety Institute. Online certifications and renewals are not accepted.
Background Check - (Background Release Form) A copy of a current (within one year) background check with clearance. Must reflect all known addresses for the past 7 years. Some facilities may require it more frequently.
OIG/EPLS Clearance Check- You must pass clearance with the Office of Inspector General and not be on the exclude list of the Excluded Parties List System.
Sexual Offender/Predator Clearance - You must clear the online check of the Sexual Offender/Predator database.
Drug Screen (10 Panel) Clearance - (Drug Test Release Form) A copy of a current (within one year) 10-panel substance (no Alcohol) test with clearance. Some facilities may require it more frequently.
Proficiency Skills Checklist - A current copy of your skilled checklist covering all specialties that you wish to be placed as. We are using a universal skill set list.
Specialty Area Test - A current test with acceptable passing test scores in the specialty areas that you are wishing to be placed.
Color Blind Test - A copy of the initial color blind test taken upon intake.
Annual Evaluation Review - Will be performed yearly near the anniversary of your application date.
Annual Orientation - Must be performed on first intake and done annually.
Annual JCAHO, HIPPA, OSHA In-Service - Must be performed on first intake and done annually.
Annual Physical - Written statement by a person lawfully authorized to perform the examination that the Employee is physically capable of performing the essential functions without restrictions. Must be performed within 1 year of start date and cannot expire during the first 13 weeks. Contact information and signature must be included for verification.
Medical Information Release - copy signed and dated.
PDD Annual TB Test for Neg. TST - Performed within 1 year of start date and cannot expire during the first 13 weeks. Must have a date placed and a date read with signature. Either a 2 step PDD or two consecutive PPDs.
Annual TB Questionnaire:
Chest X-Ray (Pos. TB Skin Test) - Performed within the first year of start date. Must be a radiology report with contact information and must include required wording. (i.e. No active disease, No active TB, No pulmonary disease). Handwritten notes may be included on the report with signature and phone number of person adding required information. Proof of a positive PPD must be provided if Chest x-ray is being used to satisfy TB requirement.

Immunizations/ Titers:
Mumps and Rubella - One MMR vaccine or positive lgG titer. We do not accept handwritten titers. Must be lab report of facility employee's history report. Immunity must be defined as Immune, Positive, or have a lab value with definition. We do accept "reactive" as results on a titer, unless it's clearly defined on the lab slip as definitive proof of immunity.
Rubeola - 2 MMR vaccines performed at least 30 days apart or positive lgG titer. If born before 1957 only one vaccine is required. We de not accept handwritten titers. Must be a lab report or facility employee's history report. Immunity must be defined as Immune, Positive, or have a lab value with definition. We do not accept "reactive" as results on the titer, unless it's clearly defined on the lab slip as definitive proof of immunity.
Varicella - 2 varivax vaccines or positive lgG titer. If only one dose is received, a titer should be performed to determine need for the second dose. We do not accept handwritten titers. Must be lab report of facility employee's history report. Immunity must be defined as Immune, Positive, or have a lab value with definition. We do not accept "reactive" as results on a titer, unless it's clearly defined on the lab slip as definitive proof of immunity.
Hepatitis B Series #3 or Hepatitis Declination Form - Signed declination or 3 vaccines (1st, 2nd 30-60 days, 3rd 6 months) or positive lgG titer. We do not accept handwritten titers. Must be lab report of facility employee's history report. Immunity must be defined as Immune, Positive, or have a lab value with definition. We do not accept "reactive" as results on a titer, unless it's clearly defined on the lab slip as definitive proof of immunity.
Influenza Vaccination or Influenza Declination Form - Signed declination or 1 vaccine or positive lgG titer. We do not accept handwritten titers. Must be lab report of facility employee's history report. Immunity must be defined as Immune, Positive, or have a lab value with definition. We do not accept "reactive" as results on a titer, unless it's clearly defined on the lab slip as definitive proof of immunity.
MAB (Management Assault Behavior) - Must be read and test completed. We will grade upon receipt and forward to facility as part of the profile. An MAB course along with certification is also acceptable.
Picture - A current head shot picture of yourself, preferred with a white background, to be placed in our staffing software and to create your ID badge. We can accept an electronic image of you, or you can send a clear picture of yourself or you can come in and we will take a picture of you.
